PRIOR TO ALL TREATMENTS

We ask that you avoid aspirin based products (aspirin, alka seltzer, ibuprofen, etc.) 5 days prior to any treatment at Med Boutique, unless otherwise recommended by your doctor. 

If you are on any blood thinners or aspirin, please alert staff before treatment.

NEW PATIENT APPOINTMENT FEE

If you are a new patient, a non-refundable $50 fee will be charged to book an appointment. This goes towards your service when you come in. If you do not show up for your appointment or are considered a late cancel (within 48 hours of the appointment), the $50 will be used to cover the NO SHOW/LATE CANCEL FEE of $50. This credit will be lost. If you reschedule ahead of time, you have 30 days to use your $50 booking fee credit.

*Insurance typically does not cover aesthetic or cosmetic services. Some surgical services we provide may be covered by your insurance, but you must check with them first

CONCIERGE APPOINTMENTS

Dr. Ashley and Janelle provide concierge appointments for days they are out of the office. There is an additional $100 fee for concierge appointments per patient.

REFERRAL POLICY

Clients who refer a new client to the practice will receive a $50 account credit to be used towards services. Accounts will be credited once the new client has paid for a service. New clients must write your name on their short information form at their initial consultation. No retroactive credits will be given.

CANCELLATION POLICY

Please let us know if you are unable to keep your appointment. IF YOU DO NOT RESCHEDULE OR CANCEL YOUR APPOINTMENT AT LEAST 2 DAYS (48 HOURS) PRIOR TO YOUR APPOINTMENT TIME, A $50 FEE WILL BE CHARGED TO YOUR CREDIT CARD. If this fee is charged to your account, it must be paid BEFORE any further appointments can be booked. We reserve the right to have patients who often violate our booking policies, to pre-pay an appointment deposit for future appointments.

OTHER POLICIES

CHILD POLICY: For the safety of children and the comfort of our guests, we ask that you do not bring children under the age of 10.


CELL PHONE POLICY: With respect for our clients, no cell phone use is permitted in our treatment area but we will certainly take a selfie with you if you ask!
